Thank you for choosing Idyllwild Pines Camp for your Retreat location.

To help you better prepare for your upcoming retreat please have the following forms ready before you arrive.

Special requested items such as a 70” T.V, portable sound system or Kitchen Box for own cooking groups, as well as your group’s final numbers (including campers and staff ratio) with dietary restrictions- Vegan, Vegetarian, Gluten-free and/or food allergies, and activity requests need to be on the Event Detail form.

Insurance & Medical Forms

Certificate of Insurance – Due 4 weeks prior to arrival.

All groups must provide a Certificate of Insurance with Idyllwild Pines listed as an “Additional Insured” on the groups general liability insurance policy (a minimum of $1,000,000.00 liability is required).

CPR/First Aid Certification – Due at time of check-in.

Idyllwild Pines requires a copy of the CPR/First Aid Certification for the group’s designated Health Supervisor

Groups with minors need a signed medical release form when on trips. We have provided you with one here, if your organization does not have one.

Staying 5 days or more?

Groups staying 5 days or more will need to provide a Group Health Supervisor who is either a Physician, Registered Nurse, or Licensed Vocational Nurse and will need the following information for campers:

Every camper, including staff and volunteers entering camp shall furnish a health history of his or her health status that is completed and signed by either the individual camper or staff member, or by the parent/guardian if camper is under 18 years of age. The history shall be kept on site with the “Group Health Supervisor” for the duration of camp. The history must include a description of any health conditions requiring medication, treatment, special restrictions or considerations while at camp, a record of immunizations including date of the last tetanus shot and record of any allergies.

Agreement to Rent Equipment

Groups planning to rent equipment from the camp such as: TV’s, projector, projector screen, sound equipment, etc. must complete this form. Items will not be issued without this form being completed.
